New Patient Registration

New Patient Questionnaire - aged 15 and under

New Patient Questionnaire - aged 16 and over

Please bring your completed registration form and new patient questionnaire to reception at any of our surgeries.  Each new patient will need to attend the surgery in person and provide photographic proof of identification (e.g. driving licence or passport).  It would also be helpful if you could bring along your previous medical card. Please keep your medical cards in a safe place. If your card is lost, please ask a member of staff for a form so that it can be replaced.

All new patients are requested to come along for new patient health check, usually with the practice nurse or health care assistant.

Medical treatment is available immediately while the registration process takes place.  Please contact reception for further information.
